Appointments are an important part of your treatment, and this time has been reserved specifically for you. Please arrive on time for your appointment. If you are more than 15 minutes late, you may be asked to reschedule your appointment.
We request a 24-hour notice if you need to cancel or reschedule your appointment. If you do not give us adequate notice, your account is subject to a $25 missed appointment fee. We are aware that unforeseen events sometimes require missing an appointment, and we appreciate your cooperation.
We are using a new computer software program called "E-Prescribing" that allows us to send your prescription for your eye drops through the computer system to your pharmacy or mail order company.
We must have a list of all medications you are taking to put on your account in the system. This will help identify any contraindications to something we may prescribe to you. Specifically, we need the name of the actual medication you are on, even if it is a generic, how often you take it, and the dosage. For example:
Lipitor, 20 mg, once a day

Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Your estimated patient portion must be paid at the time of service. As a service to our patients, we will bill insurance companies for services.
